Most visually appealing game played?

What is the best looking game you have played in terms of visuals? I am not just talking about the resolution but instead how areas are populated and such. 

I personally would cast my vote to Lords of the Fallen as been up there for me. They were able to crease some lovely areas and add some color in as well (something which most games in a medieval setting don't achieve.

I would have to go with my current favorite, Dragon Age: Inquisition.  It feels like a living, breathing world that you can interact with and watch it grow.  It doesn't have the same individuality as Skyrim and its NPCs, but one look across the valley from your castle or through the woods to the dragon fighting on the beach, and you feel like this world is huge.  It's one of the reasons I keep coming back to play it after I've beaten it.
 
I feel like Ryse: Son of Rome takes this spot easily, at least the PC version. Not only graphics but the gameplay is also really enjoyable.
For consoles I think Mortal Kombat X looks pretty slick, alongside Witcher 3.
 
I have to say that Project CARS on Xbox One looks really amazing visually.

There's an issue where if you select to race at sometime like midnight during June you are greeted with an endless sort of sunset phase, but other than that it looks really nice. Set the date for anything other than that, at around sunset time and you're in for a real treat in terms of visials. The sun hangs low in the sky, the clouds are sprayed with a mix of orange-yellow and the long shadows drift across the circuit.

Really, really amazing.
 
Oh, yes, I can also recall having utterly liked GTA 5's graphics. Just when I thought back in 2008 that GTA 4's graphics seemed surreal, this one came in 2013 and broke up every single expectation! I was personally amazed by the amount of effort they put in this game, but it was very welcome for the billions of dollars they made afterwards. I tip my hat to them.

I wish I also had a starting budget to found my own game... anyone interested? haha.
 
I'd have to go with The Witcher 2. I think that was the most impressive in terms of visuals (at least the most impressive my old PC can handle :p).

Spec Ops: The Line was good as well. They did a great job portraying the destruction of Dubai. 
 
Personally, it was Bioshock Infinite. The whole city floating in the air was just a great scenery to look at. I felt orgasmic when I played this on 4k at a friend's house, using their 42" TV.

GTA V looks even better on the next-gen console enhanced version. There are some really visially appealing aesthetics that just bring the world to life.
 
Lords of the Fallen does indeed look amazing, should check it out sometime. I played through Ryse: Son of Rome recently and was blown away by the level of detail. While I didn't quite enjoy the game, it was just lacking in so many areas. However, I found myself just stopping in my campaigns to sit and admire the scenery. Everything has been put together with such care and I shudder to think what the budget must have been for the art assets. 
 
"The best visuals" is a bit subjective because it depends on the style and mechanics. Also, a lot of people seem to misinterpret style for quality. Eg: "I think WoW is ugly because it's cartoony." So instead of saying what is the game with the best visuals I had ever seen, I will instead say a few games that visually looked pleasing to me based solely on quality in-lieu of style.

- Action FPS: Crysis 3 for beautifully rendered environments, objects and effects.
- Action FPS: Call of Duty 2 for immersion.
- RPG: Skyrim for ambiance.
- Strategy: Civilizations 5 for fluidity.
- Adventure: Don't Starve for crisp clean visuals.

There are tons more, but that's a few I would like to mention.
 
Hey, Lords of the Fallen doesn't look that bad. Anyhow, the most visually appealing game that I have played definitely has to be Bioshock: Infinite, and before I forget, Witcher 3. Bioshock Infinite has beautiful vibrant graphics and clear skies, and it just makes me WANT to visit the imaginary town of Columbia, it just looks like something straight out of a fairytale.

Anyhow, it just looks like a nice atmosphere to live in, and it just looks beautiful. 

Witcher 3 is obviously supposed to have better graphics, which it does, but it just doesn't have that nice atmosphere for me. Some of the cities are beautiful, and the graphics really are very, very good, and it's a great game and you should definitely play it if you have a good enough PC to run it. However, Witcher 3 recently got its graphics downgraded for some reason, and it looked even better before that. But hey, what can you do, it still looks great. 
 
I haven't actually played it but Bloodborne is up there. It's a combination of technically good graphics and fantastic art direction and designs. Though it's looking like Dark Souls 3 is gonna look even better than that!

BTW, Lords of the Fallen does look pretty awesome, wanna play that one!
 
My top five would be the following:

1. GTA V (Next Gen) : This game blew my mind when it came out on PS3/Xbox 360 and I couldn't believe it when it was released on next gen consoles and eventually PC. The sheer detail, the sound effects and the scale of everything is just jaw dropping.

2. Uncharted 4: A beautiful, flawlessly crafted masterpiece that makes me happy to own a PS4.

3. Witcher 3: I haven't played it, but I have watched a lot of gameplay and it looks beautiful. The world is also considerably large so I am quite impressed with the result.

4. NFS 2015: Yes, its EA garbage but it does look really nice, especially for a game that takes place at night. The cut scenes are hard to differentiate between graphics and live-action and the car customization is much more fun when you can see the car in full detail.

5. Black (PS2) : By far the best looking PS2 game to ever exist. The graphics, the effects and the realism of the gun play made this one of the best shooters to ever grace Sony's iconic console.
