Post your opinion of the scariest way to die in a video game. For me it would for sure have to be falling in lava in Twilight Princess.

It scared everyone I'm sure. I was only 5 when I first encountered that and needless to say I paused the game, ran away from my room, and didn't touch the game for a day or so.

I would say being killed by one of the chainsaw guns from Gears of War... or the chainsaw guys from Resident Evil 4. It would basically be the same death and both would be really scary.
 
So I'm playing Resident Evil 4 Wii Edition and I get to the part where you come across your first Regenerator (see below). These guys can only be killed with exact shots to certain parts of their torso.

So he's lumbering toward me and I'm getting ready to shoot him in the exact spots I need to when my Wiimote batteries start to die. This leads to Leon walking backwards in counterclockwise circles repeatedly. The screen follows Leon so I get to watch in horror as the giant magically appears closer and closer every time I turn back to face him until this happens:

Not unless it's the same fish from Yoshi's Story.

Actually, dying from the piranha plants in Yoshi's Story would be scary. I mean chapter 4 2-4 are actually really scary. When I was a kid they scared me and I played YS for the first time in a long time, and I'll admit, world 4-3 is scary as hell. The piranha plants are brutal and they eat you and make you small. Sometimes you can't even avoid them.
 
Probaly being inflated in Dig Dug. Just think about it. Slowly being pumped up with air until you finally explode.
 
Mario's death animations for drowning and dark matter are always a bit creepy in later Mario games.

Yeesh. Mario even holds his throat, struggles widly and starts floating dead to the surface, in case you needed nightmares of a cartoon plumber dying horribly.

The creepiest way to die in possibly any game however might be one I remember seeing in a Mario RPG fan game, where Mario gets executed by firing squad upon game over, complete with a cut to his funeral. That's kind of disturbing to watch.

I posted here earlier, decided I didn't like what I said but I'd already posted it, and for lack of a better idea (and a complete inability to find a delete button, which I swore I knew existed) I turned the other post to (removed). However, I've since thought of a better death than before, and one worthy of being here. I'm a sucker for the "thing leaps from seemingly nowhere and kills you" type death…I'm not a fan of horror because I jump at everything in films designed to elicit that sort of reaction. Trying to remember my original post, I think I had one of those…probably the Hunter from the Left 4 Dead games. However, in my experience, the scariest way I've ever died is what happens if you break 100 or more signs in Oracle of Seasons and go talk to the sign maniac in Subrosia. If you haven't played the game don't click the spoiler, but get it off the 3DS E-Shop and do it yourself. The Oracle games are my absolute favorite handheld Zeldas, and I recommend them to anyone who hasn't played them.

If you break 100 signs and talk to the sign maniac, he tells you that you're going to pay for not loving his passion as he does. The next thing you know you're back at the first thing you see after turning the game on and getting past the GameBoy Color load screen…the Capcom logo. It persists for a few seconds, then snaps back to the game where the guy's laughing at you at how freaked out you got. I was somewhere between the ages of 5 and 7 when I played through this the first time…I panicked. I hadn't saved in a while (a bad habit I'd had when I was younger) and I thought the guy was seriously resetting the game, causing me to lose a few hours of progress. Apparently Nintendo and Capcom worried this was too mean, though, as the guy gives you a ring because he felt bad about how freaked out Link supposedly gets. Granted it's one of the useless, commemorative ones, but it's still a nice easter egg.

…What, that wasn't an actual death? Bleh, fine. Link's Awakening…you being able to steal from the shop was great, especially with how expensive the bow was and how slow rupee gathering was in that game. However, if you steal from the shop, the next time you enter the shopkeeper reveals his insane wizard powers and lightning-zaps you into oblivion without you being able to react. Even today I do my best not to steal from the keeper because of how terrified it made my 5-7 year old self the first time it happened…plus I dislike being renamed THIEF by absolutely everyone, even the darn Wind Fish!

It's a fan game. Which makes it a bit better, but doesn't stop it being potentially traumatising.

That scream is creepy. The fact he splits in half and explodes makes it even worse.

There's also all the Dark Matter/Zero/O2 battles. Ripping the enemy's eye out with blood everywhere and then having that die in even more blood? That's dark, even for a Nintendo game.

Having never played a Resident Evil game before 'Revelations', I was kinda creeped out by how the Comms Officer chainsaws Jill to death.

Yeah. The name kind of says it all really.

Also, for a pretty disturbing fan example, the game over screen of Super Mario World Dark Horizon shows Mario in a SMW esque gas chamber, before having him shout in pain and get quite literally melted to the bone. Yet another rather disturbing part of the game, in case the Nazis, world domination, Iraq war references and presence of the Taliban as enemies weren't bad enough.
